The sachet contains important ingredients like racecadotril and saccharomyces boulardii which is used in controlling severe diarrhoea. Racecadotril (acetorphan) is an oral enkephalinase inhibitor works by preventing the degradation of endogenous enkephalins, racecadotril reduces hypersecretion of water and electrolytes into the intestinal lumen. Racecadotril treatment also results in significant reductions in stool output. Saccharomyces boulardii is true probiotic yeast. It has potent beneficial effects in the body without the risk associated with some other probiotics. Probiotics are bacteria that live in your body naturally and help your intestines break down food. Probiotics that are the same or very similar to the ones that your body makes can be taken to supplement healthy digestion.

Mechanism of Action

The active metabolite of racecadotril, thiorphan, inhibits enkephalinase enzymes in the intestinal epithelium , protecting enkephalins from being broken down by these enzymes. This reduces diarrhea related hypersecretion in the small intestine without influencing basal secretion. Racecadotril also has no influence on the time substances, bacteria or virus particles stay in the intestine.

Pharmacokinetics

Absorption:
It is rapidly absorbed after oral administration
 Cmax reaches within 60 minutes
Presence of food delays the absorption.
Volume of Distribution: 90% of the drug is bound to plasma protein
Metabolism: Mainly metabolised in liver to form active metabolite called thiorphan.
Excretion: Metabolites are mainly excreted in urine and very few amount is excreted in feces
Half-life of racecadotril is avout three hours.

Saccharomyces boulardii is a yeast Saccharomyces boulardii is most commonly used for treating and preventing diarrhea, including infectious types such as rotaviral diarrhoea in children, diarrhea caused by gastrointestinal (GI) take-over (overgrowth) by "bad" bacteria in adults, traveler's diarrhea, and diarrhea associated with tube feedings. It is also used to prevent and treat diarrhea caused by the use of antibiotics.
Saccharomyces boulardii showed reduction of relapses in some specific patients with recurrent Clostridium difficile infection and may be effective for secondary prevention of C. difficile infection

Precaution & Warning

  • This medicine is not recommended for pregnant women.
  • This medicine is not recommended for breastfeeding women.
  • The treatment should be discontinued if any symptoms of skin reactions appear.
  • Dose should be adjusted in patients with renal insufficiency.
